With access to video relay service, SRV Canada VRS is creating opportunities for Deaf Youth across Canada. A Deaf child under the age of majority can register to use the SRV Canada VRS app with the authorization of their Parent or Legal Guardian. Parents or Legal Guardians can provide authorization via Online Form or Paper Form by following the instructions below:
AUTHORIZATION VIA ONLINE FORM
Step 1: A Parent or Legal Guardian must fill in the authorization below and click submit.
Step 2: Once the authorization has been received, a member of our Customer Service team will contact the Parent or Legal Guardian to provide a parental consent form prior to the final submission process.
Step 3: Within a business day or two, the child’s 10-digit number will be sent to the email address provided and they can start using SRV Canada VRS. Please remember to download the app prior to using the new VRS number.
AUTHORIZATION VIA PAPER FORM
Step 1: To register within the SRV Canada VRS App, you must download the app first.
Step 2: As part of the registration process, you must indicate you are registering a Deaf child who is under the age of majority. By doing so, we will automatically email a Parental Consent PDF form to the email address provided for printing.
Step 3: Once the form is printed and completed, scan it and email it to Customer Service at firstname.lastname@example.org or mail the form to:

Step 4: Once the signed Parental Consent form has been received, a member of our Customer Service team will contact the Parent or Legal Guardian for confirmation purposes.
Step 5: Within a business day or two, the child’s 10-digit number will be sent to the email address provided and they can start using SRV Canada VRS.
